A binder that is provided and made by the business that can be taken to MD appointments. This binder will contain your list of medications, allergies, emergency contact information, medical and surgical history, etc. All the information will come from you, the patient. I will organize it so that it's user friendly and to help you and your MD's during appointments. There will also be an option for an ongoing monthly subscription so that the company can keep your records up to date and add any changes that come with any doctor's appointments.
